Basics
Let’s start with the basics, so when a patient comes to meet us to do some kind of gender affirmation, so we need to start with why we want to do it, since when have we had this feeling? What the question wants to clarify is, Is it something that has been there for a long-time or has something happened making this a recent development? It could be because of something that is bothering us , say we have depression or there is some external pressure because of which we want to do it. So in the very first consultation, we have to rule these things out.
So after this is done, we connect them to a psychiatrist and a psychologist, if we haven’t done this before. Because it is very important to get a psychological assessment and a psychiatric evaluation done before planning any treatment or planning any therapy, hormones or even surgery. So this is the first treatment we provide.
Second, once that process is done and we can come to treatments- surgical and non-surgical.
Surgical Procedures
In surgical procedures, there are two aspects- one is top surgery and the other is bottom surgery. So top surgery and bottom surgery are what is done on people in general. And if there is anything else then you can add to it.
MTF
So if we go from top to bottom, in MTF transition surgery. So MTF starts with-
- Hair Transplant
- Facial Feminization
- Changing the shape of your brows and forehead
- Rhinoplasty where you change the shape of your nose
- Cheekbone surgery
- Lips surgery, you can change the size here- make it bigger or smaller.
- Jawline counturing with chin surgery known as genioplasty
- Voice surgery, which is essential for some people
- Adam’s apple surgery, which is prominent in males, so people don’t want it. It should be flat.
- Then comes breast surgery, so MTF wants breast implants or fat grafting
- There can be liposuction or fat grafting for the stomach
- Same for the hips, you can increase your fat in some areas and decrease it in others to give it more of a feminine contour.
- Then comes the bottom surgery or the genital surgery which is the creation of a new vagina by taking out the existing penis and the scrotum.
- And then beneath that you can do liposuction, you can do calf implants to make your calves look more prominent.
FTM
For FTM Transition surgery, here are the procedures that you can opt for:
- You might or might not want hair transplant.
- Then there is facial masculinization, making your face more prominent with stronger jawline and cheeks or chin.
- Transplant for a beard. So this is for people who want a beard also there can be the proper contour and shape that the beard has.
- We can make your voice deeper.
- Then it is breast reduction surgery where we can decrease the size of your breasts which also has different techniques.
- Then there is liposuction to get a more masucline frame by taking out fat from certain areas and form a V shape or a rectangular figure.
- Then comes bottom surgery, which is the reverse of the former. So the uterus and ovaries are taken out. The vaginal canal is closed and a penis is formed which also has a few techniques. The most common of them is when you take your non-dominant forearm, which is mostly the left hand in people, and make the penis and urine tube, also known as urethra are formed.
- Additional procedures that happen in FTM is penile implants either inflatable or firm and scrotal implants, so that you get a proper feel of the scrotum.
