Realneilbruce This affidavit is only for gender change for name and gender change there is another format which is not with me. If you are changing your name also you can add it immediately after point 1 and if you want to mention about your medical intervention then you can mention that after point 3 and before point 4. Do remember to correct the serial number mentioned for verification if you are adding any point.

Where did you get this from? Any pointers for how change of name (and medical intervention) should be stated?

As for the name part I got the information from a lawyer
And medical intervention part I got to know from the DM office

For name change part you can contact a lawyer

If you don’t find one you can ping me in private here will share the name change format too

I wanted to ask you a question, is there a permission or any legal document required before the bottom surgery in MtF ??? Or I can undego bottom surgery only with the help of the GD letter of the psychiatrist?

usually as per the trans act you need to declare yourself as transgender and apply under the first option

and after surgery you have to apply under the 2nd category

but because I explained the act and interpreted the way I understood to the dm I was directly able to apply in the 2nd option and get f marker

Amrit Your GID and HRT letters are enough documents for SRS but some of doctors taking affidavit also. The content of that affidavit given by doctor, you may get notarized from advocate.

However I want a clarity that suppose I am on HRT for 1 year under due consultation and supervision of doctors and now my Sex reassignment surgery or bottom surgery is scheduled.

So, my question is can I go with SRS directly or I have to take permit of the magistrate of my city or present myself before him only then I will be allowed to do SRS because I read somewhere and infact in some YouTube videos also that the permission of magistrate is required before SRS.

Amrit No Never!

SRS is your choice not need any kind of permission from anybody except if your age below 18 that case only your parents support required.

surgery is a personal decision which is affirmed by the honourable Supreme Court also so you do not need to present yourself in front of any government officer. For this only thing is a few doctors might ask you to get an affidavit saying that you understand the risk of going through the reassignment surgery, but other than that, you do not need to take anyone's permission, other than your doctors recommendation, letters that you're finished one year of hormone replacement therapy.
