How to prepare the documents for GRS?
In the beginning, while gathering the documents it might seem overwhelming. But it is not a lot. Dr Richie Guota reiterates that. “We want a life-long relationship with our patients and not one that is just about their surgery.”
What is the need for a psych evaluation?
In aesthetic surgeries as well as gender reassignment surgeries, it is important that we figure out what the issue is. If it is not determined properly then it might lead to further issues.
World Association of General Health and the Indian Professional Association came out with the 7 Standards of Care by WPATH and we have to maintain those. A patient needs two different mental health professionals to consult them. They can talk to the patient and determine if there is some sort of psychopathology involved. If there are no issues then good but if the patient does have an issue then it needs to be treated. The psychiatrists also help the patient understand the details of this invasive procedure that is irreversible.
If the patient understands all of it well and still wants to go forward with it.
The mental health professionals need to be from two different institutions to ensure that the results are not tampered with.
The third thing is that HRT. While HRT is not compulsory, it is instrumental in stopping male pattern baldness. The hair becomes smoother and shinier and 22 per cent of body fat is concentrated at the breasts of cis-women. So all of this helps and the patient’s dysphoria is reduced so the patient feels better about the gender identity they want. So this is why I suggest that the patients take 12 months of HRT beforehand. But some people cannot do so because of medical reasons, so I don’t tell them. All of this is with regard to genital surgery.
For breast surgery, just two mental health professionals and their certificates would be enough. We don’t meet our patients once for the operation- we are connected to them for their entire life. This is why we tell the patients to have both consultants so that the file is kept right.
We also need an affidavit of the waiver of liability. This is a 100 rupee stamp paper which is notarised because normally a surgeon is not ethically allowed to remove normal body parts from a patient. So for example, we cannot remove testes from a transwoman- this is a major loss to their body. So this affidavit states that the patient has gender incongruence. And the patients took advice from the specialists who saw that the patient has gender incongruence and needed surgery so then they permitted us by saying they were ready to perform surgery. So they let us take their healthy organs out and change their sexual functioning. This is what they want.
Do we need mental health certification for other types of surgeries?
For irreversible and breast surgeries, a mental health professional certification is essential. But for surgeries like facial feminization or FF and hair transplant surgeries, as well as rhinoplasty- a GD certification is not needed.
