It was 2k for one session and I got the letter a couple of days after the single session (I was very clear that my intention with the appointment was to get the letter, and didn't express any doubts about being trans in session). He was respectful.
Though, if you just want to get a letter to start HRT, you want to maybe play down your other mental health issues, because there's often the opinion of 'solve those first before transitioning' that professionals have.
If you're on a tight budget the group he's with, SCARF, provides counsel for low cost. You could reach out to them and say you are tight on funds and would like to meet this doctor.
