Regarding voice surgery, there is Crycothyroid Approximation surgery (CTA) which seems to be quite popular with ENT specialists. The other option is a Glottoplasty. It seems that a Glottoplasty produces a better ,less hoarse result than the CTA. I have quite a public job and fortunately my current voice passes but am planning on having a glottoplasty so that I don't have to "think twice" when I talk. One final point:a feminine voice is not just about pitch elevation but also resonance intonation and even your lexical resource. Therefore, some kind of voice therapy is still necessary after voice surgery.
