TL;DR You need:
- A notarized affidavit certifying your change of name and gender
- A photo ID with an address in your state of residence (to which you are applying for the Transgender ID)
- A current photograph, and desired new signature
- A letter on a hospital letterhead by your treating professional certifying a GD diagnosis and gender affirming medical interventions
Once you have these, just go on to transgender.dosje.gov.in and apply. You will (or at least, are legally entitled to) hear back from them within 1 month. Depending on the state, they might either visit you or you might need to visit either the DM or some other office under the DM for verification. After this, both a physical and digital certificate will be issued.
If there are any corrections required, there is an option to "respond" by uploading revised documents via the same website, after which your application should be approved.
