Provided by the psychiatrist from what I've heard so far.
My letter gives a brief background about me (age, education, location), a summary of my experience with gender, states any significant medical, surgical or substance abuse history, says I show no evidence of any major mental illnesses and that I seem sensible and can make decisions for myself, and then goes on to say I have gender incongruence and have approval to undergo gender affirming medical and surgical interventions.
I've also seen letters that are much simpler and shorter.
Just make sure to include your legal name, preferred name, the 'gender incongruence' statment and the 'can undergo gender affirming treatments' statement. And ofc the psychiatrist's signature/stamp etc.
If you want it to double up as a go ahead for surgical interventions ask them to mention that too.
